Hospice of the East Bay – Nurse Practitioner Positon

Hospice of the East Bay (HEB) is a community based program of home care for terminally ill patients. Care is given to patients and families by an interdisciplinary group. Bereavement care is provided to the family for one year after the death. Hospice services are given regardless of ability to pay, making the agency dependent on donations and insurance billing to meet operating expenses.
Reporting directly to the Chief Medical Officer (CMO), the Nurse Practitioner/Advanced Practice Nurse for HEB provides primary care to HEB patients with appropriate physician supervision as assigned, while functioning as an integrated part of the hospice interdisciplinary team. Assists in developing and adjusting the comprehensive plan of care to optimize patient outcomes and assures alignment of care plan with best practices as well as patient care goals. Scope of responsibility includes clinical practice, education, consultation, and research. This position works interdependently with and independently of the multidisciplinary team. May provide care in both the home care (home, SNF, RCFE, ALF) environment as well as in the HEB in-patient facility (Bruns House)
Serves as primary clinician (under supervision of designated hospice physician) for patients who enroll in hospice without an assigned clinician.
Serves as a member of the Hospice Interdisciplinary Team and participates in development of a comprehensive plan of care for patients served.
Consults with the Medical Director/CMO as needed to ensure best practices.
Performs home visits as required (alternating months with MD if patient is a Skilled Nursing Facility, SNF, resident; otherwise initial visit, face to face and then as needed for symptom management).
Documents in the Electronic Medical Record (EMR) indication for, and context of all patient encounters according to HEB documentation standards as well as Medicare COPs. (supports level of care)
Performs home visits when physician/NP level visit is needed or upon request, for patients with another clinician as the primary clinician of record.
Provides telephone and on-site consultations to clinical/nursing staff regarding patient care and appropriate administrative issues as needed during scheduled hours or PRN on-call.
Reviews patient records at time of recertification, performs recertification visit and completes appropriate documentation when requested by hospice Medical Director/CMO.
Performs initial and on-going medication reviews with appropriate documentation as directed by hospice Medical Director/CMO.
Acts as primary clinician at Bruns House when needed as requested by CMO.
Presents in-services to staff at SNFs, RCFEs, ALFs, and physiciansâ€™ offices as indicated or as requested by HEB leadership.
Presents educational in-services to hospice clinical staff.
Mentors new NP hires for HEB standards and integration into HEB best practices.
Participates with the hospice physicians or Quality Assessment Performance Improvement (QAPI) team on quality improvement projects as indicated or requested.
Provides wound consultation services to patients as indicated by wound care needs and as requested by RNCM, hospice physician/CMO.
Provides call coverage for adult hospice homecare, SNF, RCFE, ALF, Bruns patients as assigned by Medical Director/CMO.
Adhere to the practice of confidentiality policies regarding patients, families, staff and the organization.
Knowledge of Medicare regulations/Conditions of Participation (COP’s) in the four levels of hospice care as they pertain to NP scope of practice within the hospice setting; excellent knowledge of fundamentals of pain and symptom management; excellent interpersonal and communication skills, demonstration of organizational skills and ability to efficiently manage schedule; must have the ability to establish and maintain cooperative relationships with professionals and lay groups; ability to work collaboratively with Medical Staff Team and as part of the HEB interdisciplinary group. Willingness to focus on hospice philosophy as it pertains to patient care delivery and to individualize the hospice care plan for each patient served by HEB: demonstrate flexibility, critical thinking and the ability to discuss news that is difficult to hear with patients and their families.
Experience & Education
Possess a current NP license with furnishing privileges in the state of California.
Master’s degree required.
Active DEA Certificate for controlled substances (schedule II and higher)
Two or more years of palliative care experience with patients at end of life.
Advanced Practice Certified Hospice and Palliative Nurse (ACHPN) Preferred
Certification in BLS and/or ALS
Current California Driver’s License
Current proof of Automobile Insurance Policy with California State Required Minimum Coverage Levels
