The post holder shall maintain a high standard of nursing care. In collaboration with the Palliative Care Medical Director(s), will develop skills in and share responsibility in assessing, developing, implementing and evaluating the holistic care of the patient and their family. The Palliative Care Coordinator shall develop the regional Palliative Care Team, carry out care as needed, develop expertise in symptom control, develop a high standard of professional and ethical responsibility and improve teaching skills. The coordinator shall work as part of a multi-professional team.
Graduate of an accredited program for Registered Nursing. Advanced degree preferred.
The nurse must have an understanding of the Mission and Core Values of CHRISTUS Santa Rosa Health Sysyem.
Ability to provide educational sessions to a variety of audiences.
Program development, implementation and evaluation.
Data collection skills to assess program progress and impact.
Current knowledge and skills appropriate to age/type of patient population served.
Knowledgeable and sensitive to patients' rights in the delivery of care.
Communicates in a clear concise manner appropriate to the developmental age of patient.
5 years nursing experience of which two years must have been in hospice or palliative care.
C. Licenses, Registrations, or Certifications
Current Texas RN license or current Compact state license.
BLS certification required.
ELNEC (End-of-Life Nursing Education Consortium) Certification required or eligible preferred.
