Advanced Practice Registered Nurses / Physician Assistants

Leads development of innovative and effective models of care delivery focused on addressing health disparities, advancing health equity and promoting readiness for value-based care arrangements
Provides clinical leadership over assigned practices and drives performance to achieve Community of Practice goals for Optimal Health, Exceptional Patient and Caregiver Experience, and Organizational Vitality.
Serves as a clinical leader advocate within the organization to address identified health disparities within the community served by ChristianaCare.
Principal Duties and Responsibilities:
Directly supervises assigned or designated clinicians and promotes their professional development.
Participates in all phases (e.g. interview, selection, etc.) of clinician recruitment
Manages physician and advanced practice clinician performance by promoting team-based care, role clarity, collaboration, and responsibilities
Serve as a core leader for the Primary Care Community of Practice through engaged and proactive communication and project/program prioritization
Addresses workplace conflicts guided by our values seeking Human Resource support as needed.
Actively participates in strategic planning for Primary Care Community of Practice expansion
In collaboration with the Primary Care Line leadership and other Clinical Directors, holds clinicians accountable for their individual and collective efforts to achieve success in defined metrics.
- Participates in data review, performance improvement projects, budget preparation, and in efforts to optimize clinical and financial performance.
Supports eBrighthealth ACO clinical and utilization initiatives.
Represents the Primary Care Community of Practice as assigned and delegated by the Primary Care Leader.
Performs other responsibilities, as assigned.
