Bedford VA is a Level 3 interdisciplinary teaching facility with service-lines serving rehabilitation, long term care, mental health inpatients and outpatients, multiple community-based outpatient clinics (CBOC), Outpatient Urgent Care/Primary Care/Specialty clinics, and Home-Based Primary Care. There is a large outpatient psychiatric population, with admissions for medical co-morbidities. Urgent Care Nurse: Responsible and accountable for all elements of the nursing process when providing and/or supervising direct patient care. Assesses, plans, implements and evaluates care based on age-specific components. Assumes responsibility for the coordination of care focused on patient transition through the continuum of care, patient and family education, patient self-management after discharge, and supporting factors that impact customer satisfaction. Considers all characteristics of the individual, including age and life stages, state of health, race and culture, values, and previous experiences. Administers medications and procedures per established policies and guidelines. Influences care outcomes by collaborating with members of the interdisciplinary team. Core elements of performance include knowledge and active participation in unit/program level quality improvement processes and initiatives as well as customer service programs. Other Urgent Care duties include but not limited too: Maintains flow of clients in the clinic by monitoring unit/patient activity and prioritizing care Understanding the Emergency Severity Index Triage Classification and document on the Emergency Department Integration Software (EDIS) board. Must be BLS and ACLS certified within months of hire and maintain ACLS certification thereafter. Completes assessments/triage in a timely manner. Responsible for coordinating patient care with other service lines. Completing unit specific reports/projects as requested. Maintains unit specific competencies, continuous educational programs Assisting Specialty Clinics as needed. Adhering to Infection Control polices as well as the proper handling of Reusable Medical Equipment (RME) equipment to include: setup, cleaning, disinfection, of instruments, and storage of instruments. Responding to Medical Emergencies throughout the facility according to MET policy Serves on unit-based committees as assigned. Participates in the orientation and educational development of staff. Work Schedule: Full Time, Monday- Friday, Variable days, 10 hour shifts 7:30am-6:00pm Financial Disclosure Report: Not required
