The Kansas City VA Medical Center is currently recruiting for one (1) Registered Nurse to join the High Reliability Service line as the Patient Safety RN. The Patient Safety Nurse is a registered nurse accountable for coordination, development and implementation of programs and activities that promote patient safety in the organization. The incumbent performs duties directly contributing to the administrative management of the facility health care delivery system, and works with the Patient Safety officer in planning, organizing, developing, managing, and evaluating patient safety activities at the facility level. Serves as the facility point of contact for VA National Center for Patient Safety (NCPS) Alerts and Advisories within the facility, including CBOC's and contract facilities providing care to veterans. Establishes a method to disseminate Patient Safety Alerts and Advisories and tracks items to completion. Maintains documentation of completed actions and ensures documentation of completion is entered on the VHA Alerts and Recalls Web site within timeframes specified. Manages the Joint Patient Safety Reporting System (JPSR) system to include: evaluation of incoming Event Reports, assignment of Safety Assessment Code (SAC) score, distributing reports to parties for additional information, and tracking action items to closure. Coordinates timely completion of Root Cause Analysis (RCA) and annual Healthcare Failure Mode Effects Analysis (HFMEA). Serves as an advisor for RCA and HFMEA teams. Tracks actions to closure and coordinates timely evaluation of action effectiveness. Conducts tracers, rounds, and investigations across the organization and communicates opportunities to local management and leadership as appropriate. Coordinates the distribution of risk management advisories and other non-VA alerts (e.g. Joint Commission Sentinel Event Alerts). Assists local staff on conducting gap analysis and action plans to meet requirements in advisories and alerts. Tracks action items to completion. Consults with staff and management on ideas for improving patient safety. Translates regulatory and accrediting body standards, policies, and strategies into action within the complex interrelationships that exist. Advises staff and management of patient safety concerns and evaluates situations for risk to patient safety. Completes annual reports and data submissions, coordinates document submissions, and tracks external review recommendations to completion. Evaluates previously closed actions for sustained compliance. Educates staff across the medical center on patient safety practices and principles.
