The VA Northeast Ohio Healthcare System is recruiting for 13 full time Nurse Practitioners. The Nurse Practitioners will function virtually within the VISN 10 Clinical Resource Hub, a repository of clinical & administrative staff that serve VA facilities within VISN 10 that are underserved or experiencing gaps in clinicians due to the inability to match provider supply with demand. Services provided by CRH staff are conducted via multiple modalities, including virtual and in-person care. Supports the Clinical Resource Hub Tele-Primary Care Emergency Department / Urgent Care Express program throughout VISN 10. Serves as a Clinical Resource Hub provider for patients requiring provider evaluation and management consistent with Clinical Resource Hub standard procedures. Performs histories and physicals. Prescription of medications within the defined scope of practice. Orders diagnostic studies as appropriate. Coordinates care within the assigned medical facility as well as with specialists and ancillary services in the community. Initiates and responds to requests for consultations, special tests, and studies. Serves as an expert in clinical practice and demonstrates outcomes at the program and practice level. Determines the appropriate delegation of tasks consistent with their obligation to provide optimal care Leads and organizes delivery-of-care to assure continuity-of-care, including access-to-care and discharge planning. Assesses, plans, implements, documents, and evaluates nursing care based on the ANA Standards of Nursing Practice, including prescribing and diagnosing. Guides staff in the application of critical thinking and process improvement to improve patient care. Anticipates and recognizes medical/surgical/psychiatric emergencies and initiates or leads others to implement appropriate and timely actions. Uses advanced clinical knowledge/judgment to promote staff involvement in planning, decision-making, and evaluating outcomes. Evaluates current practice and formulates outcomes for groups of patients within area of expertise.
