The RN is responsible and accountable for all elements of the nursing process when providing and directing patient care in the specialty medicine services. The Case Manager functions as advocate and contact person for patient, family, healthcare team members, employees and outside agencies to assure continuity of care and optimal patient, clinical and resource outcomes. This position will involve most hours being carried out in the Gold Clinic at the Omaha VA. The position requires the following skills and abilities: Strong Medical/Surgical background Demonstrated flexibility Demonstrated excellent Nurse /Provider collaboration skills Demonstrated sound judgment in decision making and ability to prioritize using the nursing process Excellent communication skills both verbal and written Excellent problem solving skills, including ability to provide case management Demonstrated excellence in nursing care delivery in all nursing areas. Strong nursing assessment skills needed Must be able to lead a team and have conflict resolution skills. Preferred experience for this position includes: Basic Life Support, Active Nursing License, and Case Management experience preferred, minimum of 2 years RN, BSN preferred. Work Schedule: Monday - Friday, 8:00am- 4:30pm. Financial Disclosure Report: Not required.
