Hospital in Home is a highly trained group tasked with assisting the Hospital and the emergency room to discharge patients back to their homes in a timely manner. The Hospital in Home RN will need organizational skills and technical skills to operate independently with a Nurse practitioner to provide treatments and interventions that would normally be done in the Hospital. Preferred background would be Home care or community care with background in Emergency medicine or ICU care The Registered Nurse in Home Based Primary Care, inclusive of the Hospital in Home (HIH) program, provides acute care services to patients in their homes. He/she assumes accountability and responsibility for his/her nursing practice and decisions affecting patient care. The position requires frequent home visits, astute clinical assessment skills and independent follow through, aimed at providing enhanced HBPC services to HIH patients. The goals of service include acute care within the Veterans residence, decreased hospital LOS, decreased utilization of emergent services, improved patient satisfaction, and fiscal responsibility. The Registered Nurse is responsible for, but not limited to the following duties: Providing outpatient clinic services to patients in their homes Assumes accountability and responsibility for his/her nursing practice and decisions affecting patient care Administers medications and procedures per established policies, protocols and guidelines. Functions as a member of an interdisciplinary team providing patient care in the home. Providing clinical oversight for LPN's on the team In home physical assessments Point of Care testing. Providing wound care, catheter care, IV therapy and phlebotomy within patient's homes. Planning, intervening and evaluating indicated actions; IV therapy and phlebotomy within patient's homes. Functions as a case manager and coordinates the patient care with interdisciplinary team members and nursing staff Other duties as assigned Work Schedule: The HIH nurse will be assigned a 7:30 AM - 4:00 PM shift including rotating weekend shifts, The Hospital in Home group is responsible for coverage 7 days per week including Holidays. Hospital in Home RN is in the field interacting with patients 75% of the time with 25% spent on timely documentation. TRAVEL: Will travel in a government vehicle (40-mile radius) and perform all duties of the registered nurse. A clean and valid driver's license is required for this position.
