The VA Maryland Health Care System (VAMHCS) is a Level I tertiary multi campus teaching facility affiliated with the University of Maryland, School of Medicine and over 100 educational institutions. The VAMHCS consists of the acute care Baltimore facility; the multi building Perry Point campus ; the Loch Raven campus ; and six urban and rural outpatient clinics. The PACT Care team ( PACT) LPN serves the essential role of being the first direct care clinician to make contact with the veteran at his/ her appointment. The LPN checks the vital signs, weighs the patient, interview/complete patient clinical reminders, administer vaccinations and other injections, and communicates any relevant patient information to the medical provider. In addition, the LPN performs point-of-care ancillary lab testing for glucose, hemoglobin A1C, PT/INR, and 12-lead EKG when needed. The LPN interacts with patients and follow-up health promotion teaching, such as management of hypertension and diabetes. In addition, the LPN facilitates patent flow and efficiency.
