The RN will work at the Durham VA Medical Center as a Referral Coordinator on an Initiative to streamline the scheduling of referral appointments. This change shifts the work of multiple clinical staff members to Referral Coordination Teams (RCT) of administrative and clinical staff dedicated to the Referral Coordinator Initiative (RCI). The RCI will deliver: Timely access to care, healthcare options and communication for a positive Veteran experience; collaborating a healthcare partnership between the Veteran and their RCT; and have a dedicated referral team of staff committed to educating Veterans about their options for care. The Referral Coordinator (RC) will work with the team members to coordinate the referral process across Primary Care, Mental Health, and Specialty Medicine/Surgery Care services, improving the experience for each Veteran.
