Transitional Care Unit
Serves multiple diagnoses including neuro, ortho, oncology, amputee, spinal cord injury, transplant, etc.
Post-Acute PT, OT, SLP.
Approximately 50 therapy and tech staff at the 2 post-acute units
Highly Collaborative between all disciplines along with MD/Physiatrist, rehab nursing, social work, dietary, etc. with primary goal to get patients home.
This position will be working 48 hours every two weeks on the rotating day/evening shift, 8 hour shifts and every other weekend. The position is eligible for benefits including loan repayment.

Demonstrates Clinical Competence in utilization of FIM Instruments
Works effectively with all members of the interdisciplinary patient care team
Demonstrates knowledge of rehabilitation principles and supports philosophy of inpatient rehabilitation care
