The incumbent is a self-directed professional licensed practical nurse who serves as the Women's Health Clinical Navigator for the Clement J Zablocki VA Healthcare System and affiliated Community Based Outpatient Clinics and North Clinics. The incumbent is crucial for helping women veterans efficiently and compassionately navigate expanding gender specific care through the VA and Non-VA Care Systems. WHCN is responsible for assisting in planning, coordinating, and performing a wide range of case management, clinical, administrative, program, and technical functions to support quality gender-specific services for women Veteran patients. The incumbent works directly with patients and closely with internal and external customers, including the Women Veterans Program Manager (WVPM), Women's Health Clinical Coordinators, Primary Care PACTs, Non-VA Community Care Team, Radiology Department, Performance Improvement, and community vendors, to coordinate clinical patient care as well as monitor and track quality care to meet or exceed national or VA standards for gender-specific services. The position is supervised under the Chief of Staff, with assignment to the Women's Veteran Program Manager, and is located in the Woman's Resource Center in Building 109. Major Duties include but not limited to: Coordinate clinical patient care through receiving the initial VA consult referral for Non-VA Care Maternity; Mammogram; Gynecological; and IVF Care. Answer questions regarding authorized services rendered through women's health. The WHCN creates and maintains a clinical database and tracking through consults, mammogram reports, BIRADS, Cervical Pathology reports, Cytology, and extraction of data from patient records. Communicates all abnormal results to Women's Health Cervical Care Coordinator and PCP for appropriate and timely follow up. The WHCN works with the women's health team and VA QI on improving performance measure outcomes through data review and action planning. The WHCN collaborates with women's health team and Community Care to review/assist with all Mammography, Gynecology, Maternity, and IVF consults, on scheduling with the patient and the community provider. WHCN completes consult with appropriate CPRS note, entering and completing the VA clinical reminder and alerts ordering provider that results are available. The WHNC improve clinical outcomes of preventive care by contacting patients who fail to keep mammogram and cervical cancer screening appointments to determine and resolve obstacles Veteran may have with keeping appointments. The WHCN ensures that patients receive notification of mammogram and cervical cancer results as outlined in VHA Handbook 1330.01 and in accordance with local policies. The WHCN contacts women Veterans who are delinquent with breast and cervical cancer screening guidelines or are coming due to facilitate timely referral for screening. All Other Duties as Assigned.
