Registered Nurse - Home Based Primary Care (HBPC)
The Registered Nurse Home Based Primary Care (HBPC) is a core member of the HBPC interdisciplinary team, functions in the PACT team role as a care manager. The Home Based Primary Care Staff Nurse provides consultation and guidance and acts as a liaison to staff, home health agencies and other professionals regarding patient needs in the community/home setting and in the interpretation of VA rules and regulations. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
