Registered Nurse - Facility Care Coordination and Integrated Case Management Coordinator
SummaryThe Inpatient Care Coordinator (ICC) is responsible for the seamless transition of patients from the acute to the most appropriate post-acute level of care. ICCs provide inpatients with education on disease process - inpatient tests - and discharge instructions. Assist providers with placement of consults and review charts to ensure there are no barriers to a timely and safe discharge. The ICCs lead the interdisciplinary team rounds and work closely with all members of the interdisciplinary team.QualificationsBasic Requirements:English Language ProficiencyIn accordance with 38 U.S.C. 7403(f) - no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English
